RESUMO

Long noncoding RNAs (lncRNAs) play a critical role in the regulation of atherosclerosis. Here, we investigated the role of the lncRNA growth arrest-specific 5 (lncR-GAS5) in atherogenesis. We found that the enforced expression of lncR-GAS5 contributed to the development of atherosclerosis, which presented as increased plaque size and reduced collagen content. Moreover, impaired autophagy was observed, as shown by a decreased LC3II/LC3I protein ratio and an elevated P62 level in lncR-GAS5-overexpressing human aortic endothelial cells. By contrast, lncR-GAS5 knockdown promoted autophagy. Moreover, serine/arginine-rich splicing factor 10 (SRSF10) knockdown increased the LC3II/LC3I ratio and decreased the P62 level, thus enhancing the formation of autophagic vacuoles, autolysosomes, and autophagosomes. Mechanistically, lncR-GAS5 regulated the downstream splicing factor SRSF10 to impair autophagy in the endothelium, which was reversed by the knockdown of SRSF10. Further results revealed that overexpression of the lncR-GAS5-targeted gene miR-193-5p promoted autophagy and autophagic vacuole accumulation by repressing its direct target gene, SRSF10. Notably, miR-193-5p overexpression decreased plaque size and increased collagen content. Altogether, these findings demonstrate that lncR-GAS5 partially contributes to atherogenesis and plaque instability by impairing endothelial autophagy. In conclusion, lncR-GAS5 overexpression arrested endothelial autophagy through the miR-193-5p/SRSF10 signaling pathway. Thus, miR-193-5p/SRSF10 may serve as a novel treatment target for atherosclerosis.

RESUMO

Depression is a common psychiatric symptom in cerebral small vessel disease (CSVD), which has a certain relationship with impairment of cognitive function and can significantly increase the mortality and morbidity of CSVD patients. The occurrence of CVSD-associated depression is less related to psychological stress, but is associated with the impairment of the brain's emotional circuit. This article reviewed the correlation between the imaging features of CVSD and the occurrence and development of depression in recent years, and the neuroimaging mechanism of depression associated with CVSD. Many literatures have shown that deep white matter hyperintensities and asymptomatic lacunar infarction in the basal ganglia are independent risk factors for depression in CSVD, and the reduction of local brain volume is associated with depression. The neuroimaging mechanism of depression associated with CSVD suggests that the occurrence of depressive symptoms is related to the neural circuits in the lobar cortex-subcortical limbic area. These findings provide clues for exploring the neuropathological mechanisms and specific treatment methods of depression associated with CVSD.

OBJECTIVE:To explore th e applicatio n of team situatio nal simulation education and teaching mode in clinical pharmacy teaching. METHODS :A total of 60 clinical pharmacy interns were selected as the research objects ,and course disease was type 2 diabetes mellitus. Thirty interns were randomly selected as control group ,using traditional teaching mode ;other 30 interns were selected as trial group ,which carried out team situational simulation education and teaching mode. The teaching effects were evaluated by using the satisfaction of interns to the two modes ,the comprehensive score of graduation examination and the self-evaluation of learning effect. RESULTS :Compared with traditional teaching mode ,team situational simulation education and teaching mode was conducive to stimulate the learning interest of interns ,improve their interpersonal communication ability , cultivate teamwork spirit ,improve the awareness of humanistic care ,and cultivate the professional attitude of clinical pharmacists (P<0.05). Compared with control group ,the comprehensive score of trial group was dominantly increased (P<0.001),and the scores of professional quality ,humanistic care and communication skills in the trial group were significantly higher than control group(P<0.01). In terms of self-evaluation of learning effect ,except for the pathogenesis of type 2 diabetes and the commonly used treatment regimens ,the self-evaluation scores of the other items in trial group were significantly higher than control group (P<0.05 or P<0.01). CONCLUSIONS :Team situational simulation education and teaching mode is superior to traditional teaching mode for clinical pharmacy teaching.

RESUMO

Cerebral venous thrombosis is a rare type of cerebrovascular diseases. Its etiology and risk factors are different from those of cerebral artery occlusive disease, which are mainly related to prethrombotic state caused by various reasons. The clinical symptoms are complex and diverse, lacking of characteristic manifestations. Neuroimaging examination is the main basis for the diagnosis. Magnetic resonance imaging/magnetic resonance venography is the best way to diagnose the disease and follow-up the patients′ condition. At present, the management of the disease is mainly aimed at etiology treatment and vascular recanalization treatment, of which anticoagulation is the most important means, and there is still a lack of recognized effective measures in prevention.

Objective Investigating alterations of axon microstructures in white matter lesions in a rat model of hypertension. Methods Eighteen male Sprague-Dawley rats were randomly divided into sham operation group (n=9) and operation group (n=9). Operation group received two kidneys two clips surgeries and bilateral common carotid arteries ligations. In the 12th week after common carotid arteries ligation, rats were evaluated by Morris Water Maze test and then sacrificed for evaluation of pathological features, including small arteries' pathologies, white matter lesions, glia changes and axon micro organizations. Results Morris Water Maze test showed that escape latencies was significantly higher in operation group than in sham operation group. The number of times of cross-over cite in the target quadrant was significantly lower in the operation group than in sham operation group (P<0.05). Compared with sham operation group, operation group showed a series of pathological features, such as arteriosclerosis, leukoaraiosis, loss of oligodendrocyte and disorganized paranode. Conclusions White matter lesion is a chronic progressive disease which involves both demyelination and axon injuries.

Objective To investigate the risk factors for malignant middle cerebral artery infarction (MMI) in different age groups.Methods Patients with middle cerebral artery infarction were analyzed retrospectively.They were divided into a young and middle-aged group (< 60 years) and an elderly group (≥60 years).The demography and vascular risk factors,stroke etiologies,baseline blood pressure,drug use before onset and common blood test results in patients with MMI and non-MMI were compared in general cases and each age group.Multivariate logistic regression analysis was used to determine the independent risk factors for MMI.ResultsA total of 912 patients with middle cerebral artery infarction were enrolled,including 299 females (32.79%) and 613 males (67.21%);401 young and middle-aged patients (43.97%) and 511 elderly patients (56.03%);159 patients in the MMI group (17.43%),and 753 (82.57%) in the non-MMI group.Multivariable logistic regression analysis showed that hypertension (odds ratio [OR]6.962,95% confidence interval [CI] 1.349-35.934;P=0.021) and NIHSS score (OR 1.551,95% CI 1.384-1.737;P <0.001) were the independent risk factors for MMI.Subgroup analysis showed that NIHSS score (OR 1.402,95% CI 1.239-1.588;P<0.001) was an independent risk factor for MMI in young and middle-aged patients;hypertension (OR 10.752,95% CI 1.213-95.295;P =0.033),diastolic blood pressure (OR 1.080,95% CI 1.002-1.164;P=0.044),and NIHSS score (OR 1.504,95% CI 1.281-1.765;P< 0.001) were the independent risk factor for MMI in the elderly patients,while systolic blood pressure (OR 0.938,95% CI 0.893-0.986;P =0.011) was an independent protective factor.Conclusions The baseline NIHSS score was an independent risk factor for MMI in each age group.Hypertension and diastolic blood pressure were the independent risk factors for MMI in the elderly patients,while systolic pressure was an independent protective factor.Therefore,timely NIHSS assessment and appropriate hypertension management had important significance for the prevention and treatment of MMI.

OBJECTIVE:To analyze the safety of linezolid (LZD) for elderly patients,and probe into the regularity of adverse reaction (ADR).METHODS:In retrospective study,elderly patients treated with LZD injection or tablets were selected from our hospital during Jan.2014-Dec.2015.The occurrence of ADRs were analyzed statistically.RESULTS:A total of 126 patients were included,the age was from 65 to 94 year-old,and the mean duration of treatment was (9.47 ± 6.31) d (ranged 1-35 d).Forty-five cases suffered from ADR after medication (35.71%),followed by 39 cases of thrombocytopenia (30.95%),9 cases of leukocytopenia(7.14 %),7 cases of hemoglobin reduction (5.55 %),6 cases of nausea and vomiting (4.76 %),4 cases of transaminase elevation (3.17%),4 cases of diarrhea (3.17%),2 cases of lactic acidosis (1.59%),2 cases of skin rash (1.59%) and 1 case of drug fever (0.79%).The incidence of thrombocytopenia in ICU patients was significantly higher than non-ICU patients,with statistical significance (P=0.019).There was no significant difference among different dosage forms of LZD without statistical significance (P=0.218).CONCLUSIONS:Clinicians and pharmacists should closely monitor the application of LZD in elderly patients,pay more attention to ADR induced by LZD,especially in ICU,it induces the high incidence of thrombocytopenia.Meanwhile,it is suggested to monitor serum lactate concentrations regularly during treatment.

Objective To investigate the correlation of cerebral small vessel disease with retinal vascular network parameters using fully automatic retinal image analysis of fundus photographs. Methods A total of 121 patients undertak?ing fundus photography in a sitting position were included. They were divided into either a cerebral small vessel disease group (n=80) or an artery atherosclerotic cerebral infarction group (n=41) according to their cerebral MRI findings. The clinical data and retinal vascular network parameters were compared between the two groups. We used logistic regression to analyse risk factors of the small vessel disease. Results The percentage of males, the incidence of smoking and serum uric acid level in the small vessel disease group were lower than those in the artery atherosclerotic cerebral infarction group(P<0.05). The bifurcation coefficient and asymmetry index of venule in the small vessel disease group were lower than those in the artery atherosclerotic cerebral infarction group(P<0.05). Logistic regression analysis showed that after adjusting vascular risk factors, decreased asymmetry index of venule was associated with cerebral small vessel disease (OR=1.16,95% CI:1.05-1.38,P<0.05). Conclusions The decreased asymmetry index of venule is associated with small vessel disease which can be used as an early diagnotic indicator.

Objective To investigate the clinical and imaging characteristics in cryptogenic stroke with right-to-left shunt (RLS). Methods Fifty-two patients with cryptogenic ischaemic stroke were included in the study and divided into two groups according to transcranial Doppler (TCD) bubble test: RLS group (twenty-five patients) and non-RLS group (twenty-seven patients). The demographic data, traditional risk factors of stroke and characteristics of le?sion patterns were compared between two subgroups. Results There was no significant difference between the groups in age or sex ratio. The percentage of patients with no risk factors was significantly higher in RLS group than non-RLS group (44%vs. 14.8%, P=0.015). The lesion was more frequently observed in the vertebrobasilar artery territory in RLS group (56%vs. 14.8%, P0.05). Conclusions Vertebrobasilar Stroke without traditional cerebrovascular risk factors is more likely to be RLS-associated, which requires an advanced TCD bubble test to find the potential cause of stroke.

Objective To study the clinical features of acute limited neck motion in children and its differential diagnosis, and to improve doctors' awareness and the early diagnosis of the disease.Methods A total of 420 patients with acute limited neck motion as the chief complaint and atlantoaxial rotary subluxation as the first diagnosis between January 2005 and January 2014 in Nanjing Children's Hospital Affiliated to Nanjing Medical University were enrolled.The clinical manifestations, imaging and consultation data of the 420 patients were retrospectively analyzed.Results There were totally 248 male patients and 172 female patients.The ages ranged from 2 to 14 years old(the mean age was 7.2 years old) ,the course ranged from 2 hours to 2 days.Among 420 patients,353 patients were diagnosed as atlantoaxial rotatory subluxation.Forty-two patients were diagnosed as cervical intervertebral disc calcification.Seven patients were diagnosed as Tourette syndrome.Four patients were diagnosed as intracranial tumors.Three patients were diagnosed as cervical eosinophilic granuloma.Two patients were diagnosed as cervical spinal tuberculosis.Two patients were diagnosed as viral encephalitis.Two patients were diagnosed as benign paroxysmal torticollis.Two patients were diagnosed as Kawasaki disease.One patient was diagnosed as neuroblastoma.One patient was diagnosed as hemophilia A.One patient was diagnosed as drug adverse reaction of metoclopramide.According to Fielding clinical classification,204 patients were type Ⅰ, 122 patients were type Ⅱ,21 patients were type Ⅲ, and 6 patients were type Ⅳ.Conclusions Based on disease distribution of acute limited neck motion in children, atlantoaxial rotatory subluxation seems to be the predominant while non-atlantoaxial rotatory subluxation is not uncommon.Differential diagnoses should be considered to avoid misdiagnosis.For atlantoaxial rotatory subluxation, conservative treatment including continuous mandible occipital belt traction and brace has been proved to be very effective methods.

Objective To evaluate the current situation and problems of the application of modified Rankin scale (mRS) in the outcome assessment in Chinese stroke trials. Methods Randomised and quasi-randomised controlled tri?als on stroke therapy published before September 2013 in 3 Chinese databases were included. All clinical trials applied mRS as the method of outcome assessment. Subarachnoid hemorrhage and transient cerebral ischemia were excluded. Types of stroke, statistical methods used for data analysis, duration of follow up, blinding of outcome assessment, types of intervention and the significance of the results were evaluated. Results Two hundred and ninety-eight trials were includ?ed in this analysis. 71.14%was for ischemic stroke, 21.48%for hemorrhagic stroke, 7.38%for both ischemic and hemor?rhagic stroke and 91.28%was for acute stroke(onset time<14d). Regarding to statistical methods used for data analysis, 50.00%of the trials used t-test or variance analysis which treated the mRS score as continuous data, while 22.15%used rank sum test or Chi-square test which regarded the mRS score as ranked data or multiply variable data. Dichotomous data was applied in statistical analysis accounts for 25.50%of trials. 12.42%trials applied mRS with other scales as the methods of outcome assessment. Duration of follow up ranged from 10d to 2 years (median 90 d, interquartile range 30-90 d). Only 5.03%assessed outcome blindly. 60.07%of the trials were drug therapy, 7.72%was rehabilitation thera?py, 10.40%were surgical treatment, 14.43%were combined therapy, 2.35%were management mode, 0.67%were nurs?ing, and 4.36%other therapy. Results in 86.91%of the trials were favorable to the tested interventions. Conclusions In aspects of, there is large difference between domestic and foreign clinical stroke trials in methodology of mRS including duration of follow up, blinding of outcome assessment and statistical methods used for data analysis.

Objective To investigate the effect of peroxisome proliferator-activated receptor γ (PPARγ) agonist pioglitazone on white matter lesions and spatial memory function in chronic hypertensive rats.Methods Thirty-six male Sprague-Dawley rats were randomly divided into a sham operation group (n =6),a hypertensive group,(n =15),and a pinglitazone group (n =15).A model of stroke-prone renovascular hypertensive rat (RHRSP) was induced by the two-kidney two clip method in the hypertensive group and pioglitazone group.At 8 weeks after procedure,the rats were administered intragastrically.10 mg/(kg · d) pioglitazone was given in the pioglitazone group and an equal volume of saline was given in the hypertensive group.Blood pressure was monitored via caudal artery every 2 weeks before procedure,before administration,and after administration,respectively.After 12 weeks of continuous administration,Loyez staining was used to observe the degree of leukoaraiosis,and Morris water maze test was used to detect spatial memory function.Results After modeling,the blood pressure of RHRSP increased gradually.It was significantly higher than the sham operation group (all P =0.001).There was no significant difference in blood pressure between the hypertensive group and the pioglitazone group (P =0.897).The Morris water maze test showed that escape latencies of the sham operation group and the pioglitazone group were significantly shorter than the hypertensive group (all P <0.05).The numbers of crossing hidden platform in the 3 groups of rats were 5.200 ± 1.798,4.560 ± 1.592,and 2.333 ± 1.978 times,respectively.There were significantly differences (F =8.143,P =0.001).Both the sham operation group and the pioglitazone group were significantly more than the hypertensive group (all P <0.05).Loyez staining showed that the grades of white matter lesions in corpus callosum of the sham operation group,the hypertensive group,and the pioglitazone group were 0.333 ± 0.516,2.600 ± 0.507,and 0.500 ± 0.522,respectively.There were significantly differences among the 3 groups (F =25.652,P =0.000).The grade of the sham operation group and the pioglitazone group was significantly lower than the hypertensive group (all P< 0.05).Conclusions PPARγ agonist pioglitazone may protect the spatial memory function by relieving white matter lesions of the chronic hypertensive rats.

Objective To investigate the level of serum cancer antigen 125 (CA125) and lactic dehydrogenase(LDH) and clinical features of patients with non-Hodgkin lymphoma(NHL), and the correlation between the level of serum CA125 and LDH. Methods Forty patients with NHL were studied and the variances of the serum CA125, LDH level before treatment were analysed. Results A higher serum of CA125was associated with clinical phase,group,hydrothorax and ascites,intemational prognostic index (IPI), and had weaker positive correlation to LDH. Conclusion Measurement of serum CA125 had important clinical significance to the patients with NHL especially to the patients with hydrothorax and ascites.

BACKGROUND:Most foreign and domestic studies on the effect of androgen receptor(AR) on cardiovascular system are limited to myocardium.However,cardiac nervous system plays an important role in the cardiac functional regulation.OBJECFIVE:To observe the AR protein and mRNA expressions in intracardiac ganglia of male orchidectomized rats,to investigate whether androgen affects the expressions,and to make a comparison with normal rats and testosterone-substituted rats after orchiectomy.DESIGN:A randomized controlled experiment.SETTING:Department of Anatomy,Medical College,Wuhan University of Science and Technology;Department of Anatomy,Tongji Medical College,Huazhong University of Science and Technology.MATERIALS:This study was carried out in the Department of Anatomy,Medical College,Wuhan University of Science and Technology between January and May 2003.Sixty healthy adult male Wistar rats,with body mass of 150 to 250g,were provided by the Animal Experimental Center,Tongji Medical College,Huazhong University of Science and Technology.METHODS:Sixty rats were randomized into 3 groups:①Control group,in which,testicles of rats were untouched.②Orchiectomy group,in which,bilateral testicles of rats were dissected.③Testosterone group,in which,the orchidectomized rats were injected with testosterone,0.2 mg once,once every 3 days,3 weeks totally.AR protein and its mRNA expressions in intracardiac ganglia of male orchidectomized rats were detected by immunohistochemical method and in situ hybridization method.MAIN OUTCOME MEASURES:The positive rate of AR-positive cells in intracardiac ganglia (the number of positive cells/the number of total cells×100%)and its mean absorbance.RESULTS:All the 60 rats were involved in the final analysis.AR-positive nerve cells were found in all the intracardiac ganglia of atrial posterior wall in the 3 groups;The positive rate of AR protein and mRNA positive nerve cells in intracardiac ganglia and its mean absorbance were significantly lower in the orchiectomy group than in the control group and in the testosterone group (P<0.05).But no obvious differences in the positive rate and mean absorbance existed between testosterone group and control group(P>0.05).CONCLUSION:AR exists in the intracardiac ganglia of atrial posterior wall,and it is regulated by androgen.

BACKGROUND: Ghildren refractory clubfoot includes idiopathic multiple contracture, untreated idiopathic clubfoot in older children and failure cases of repeated operations. It is very difficult to treat this type of children patients. It has important significance in the exploration of new therapeutic methods.OBJECTIVE: To improve the therapeutic effectiveness of refractory clubfoot in children with Ilizarov method.DESIGN: a self-controlled study.SETTING: Department of Orthopedics, Nanjing Children' s Hospital, Nanjing Medical University.PARTICIPANTS: Totally 3 cases of children idiopathic multiple contracture, 2 cases of idiopathic clubfoot in children aged more than 10 years old without treatment and 6 cases of postoperative recurrent idiopathic clubfoot were admitted by the Department of Orthopedics of Nanjing Children' s Hospital Affiliated to Nanjing Medical University during 1999 to 2000. All cases manifested abnormal ankylosis and shortening deformity in foot.METHODS: Twelve refractory clubfeet in eleven patients were treated with the Ilizarov method. After achilles tendon lengthening and posterior release,kieschner' wires or olive wires with tension were penetrated through multiple planes of the middle lower 1/3 of tibia, calcaneus and the 1st to the 5th metatarsals, which were fixed respectively to external bone fixer of a ring,half-ring or horseshoe shape and connected with each other by multiple screw bars into a three-dimensional external bone fixing set. Plantarflexion, inversion, adduction and shortening deformities were corrected by the adjustment of the distance between each part of the external bone fixing set through extension, compression, and rotation, etc., and navicular and cuboid osteotomy.MAIN OUTCOME MEASURES: All 11 cases were followed up for 2 to 10 years with an average period of 4. 5 years. Indices like foot morphology,talocrural articualr activity range and deformity correction were observed during the follow up.RESULTS: The results were excellent in 5 cases (6 feet), good in 4 cases (4 feet), fair 2 cases(2 feet) according to the Garceau standard scale. The rate of excellent and good clinical results was 83.3%. Mild forefoot adduction was left in 2 feet of 2 cases and quite obvious tarsal articular ankylosis was found in 4 feet of 4 cases; however, there were no significant differences of gait and load bearing between these patients and normal children.CONCLUSION: The Ilizarov technique is a safe and effective method for the treatment of refractory children clubfoot while its indications should be controlled strictly.

<p><b>BACKGROUND</b>To evaluate the clinical effect and toxicity of NVB and DDP combination chemotherapy in the treatment of advanced non-small cell lung cancer.</p><p><b>METHODS</b>All 52 patients were treated with NP regimen: NVB 25 mg/m² iv, D1, D8; DDP 80 mg/m² iv, D1. The regimen was repeated every 3 weeks, tumor assessment was done after 2 cycles.</p><p><b>RESULTS</b>There were 2 complete responses (CR) and 26 partial responses (PR). The overall responses rate (CR+PR) was 53.8%, and the main toxicity was leucopenia, nause, vomiting and phlebitis.</p><p><b>CONCLUSIONS</b>The NP regimen is effective in the treatment of advanced non-small cell lung cancer, the toxicity is well tolerated. RESUMO

<p><b>BACKGROUND</b>To evaluate the response, adverse effects and survival of MVP regimen and TVP regimen.</p><p><b>METHODS</b>Sixty six patients with advanced non-small cell lung cancer were randomized into two groups:MVP arm (32 patients, mitomycin C 6-8 mg/m² d1, vindesine 2-3 mg/m² d1 and d8, cisplatin 70-80 mg/m² d1) and TVP arm (34 patients, pirarubicin 40-50 mg/m² d1, vindesine and cisplatin were the same as arm MVP). Characteristics of the patients were similar in two arms. All patients received two to four cycles of chemotherapy.</p><p><b>RESULTS</b>The overall responses were 34% (11/32) in the MVP arm and 56% (19/34) in the TVP arm. There were 1 complete response, 10 partial responses in the MVP arm and 1 complete response, 18 partial responses in the TVP arm. TVP regimen appeared to have a higher objective response, but no statistically significant difference in the response was observed between two regimens (Chi-square=2.269, P=0.132). Main side effects were hematological toxicities. Grade III+IV hematological toxicities were significantly higher in the patients of arm TVP than arm MVP, especially neutropenia (79% vs 44%, Chi-square=7.458, P=0.006). Median survival time was 12 months vs 8 months, and 1-, 2-, 3-year survival rates were 53% vs 24% (Chi-square=4.943, P=0.026), 17% vs 6%, 6% vs 0, for arm TVP and arm MVP, respectively..</p><p><b>CONCLUSIONS</b>MVP regimen has a lower response rate and longer survival time but less hematological toxicities than TVP regimen. The results suggest MVP regimen is a safe and active regimen for advanced non-small cell lung cancer.</p>

We Injecte(?) 12-16 ?l 20—40% HRP in normal sanne into the myocardium orthe anterior wall of the left ventricle in six rats and 10 ?l choleragenoid-horsera-dish peroxidase conjugate(CB-HRP)in three rats.The Th1-3 DRG and the nodoseganglia of both side were removed.The sections of these ganglia were proceeded bythe TMB chromogentic reaction for HRP and immunohistochemical reaction(the firstantibody is the substance P antiserum).There are three types of labeled cells——theHRP labeled cells.Sp-IR cells,and HRP-Sp-IR double labeled cells were observedin the Th1-3 DRG and nodose ganglia of both side.The parts of Sp-IR nerve fib-ers in the heart wall originate from the DRG and nodose ganglia and these neuronsprojecting the HRP-Sp-IR nerve fiber contained substanse P.Their functions may berelated to the pain(nociceptive)sensation of the heart.This study may also be aevidence of the main function of the cardiac sympathetic afferent fiber is the con-duction of the pain sensation.A few of HRP-Sp-IR double labeled cells in thenodose ganglion observed suggest that the cardiac parasympathetic afferent fibermay participate in conduction of the pain sensation.This question requires furtherstudy.
